Apply for ration card
Brief Information: Ration card is an approved document issued by the government authorities for availing benefits under PDS. Before the implementation of NFSA'2013 in the state, there were three types of ration cards in Assam, which were called Family Identity Cards (FICs). These three types were for ration cards
I) APL (Above Poverty Line)
II) BPL (Below Poverty Line)
III) AAY(Antoday Anna Yojana) beneficiaries under PDS/TPDS.
There was also another type of ration card issued by the state government called the MMASY ( Mukhya Mantri Anna Suraksha Yojana) Card.
With the implementation of NFSA’13 in the State, the State Government abolished all earlier ration cards and issued only two types of ration cards viz.
1) Antyodaya Anna Yojana (AAY) Ration Card
2. Priority Household (PHH) Ration Card
Who can apply for a ration card under NFSA:
1. Persons without Ration Card: Senior women in families with an annual family income of less than Rs. 1,00,000/- (Rs. 1 lakh) in this country can apply for a new ration card under NFSA'13, if the person has N meets all criteria to qualify as a beneficiary under FSA'13. A senior male person can apply for the same in the absence of any adult female member in the family. In this case, the person shall produce a written certificate from the concerned Gaon Murbi/Gaon Panchayat President/Ward Commissioner/FCS&CA/Inspector of the Authority that the person does not have a ration card. Such a certificate will normally be issued after the person files an affidavit to this effect with the issuing authority. This document should be attached to the ration card application form.
2. Duplicate Ration Card: If the ration card is lost or mutilated, defaced, illegible or otherwise rendered illegible unless a duplicate ration card cannot be issued by the competent authority after charging the appropriate fee without any fault of the ration card holder.
3. Persons holding ration cards in previous places: This applies to persons transferred from one place to another. This requires a certificate of surrender from the relevant FCS&CA authority where the person previously resided. This document should be attached to the ration card application form.
4. Ration card must be entered after marriage: In this case, a deletion certificate is also required from the previous residence, which is issued by FCS&CA authorities. This document should be attached to the ration card application form and ration card.
5. In case of birth of the child: Addition to the ration card requires a birth certificate of the child along with the ration card where the addition should be done with a neat handwritten or typed application on plain paper addressed to FCS&CA authorities.
Where to apply for the issuance of a ration card?

- Details of family members
- Copy of Birth Certificate of Minor Member(Under 10 years)
- Attested copy of the relevant page of the electoral roll
- Copy of Tax Pay / Land Revenue Pay Receipt
- Surrender proof of ration card/family identity retention or unavailability certificate from the FCS&CA authority where the applicant previously resided.
- Address Proof (Certified Copy of PAN Card /Driving License / Bank Passbook / Post Office Passbook /Municipal Receipt / Electricity Bill / Telephone Bill)

- Self-declaration
- Police report
Documents required for separate ration card
- originally surrender certificate(s).
- Basically the parent's ration card

- Certificate from Gaon Burha/Gaon Panchayat President/Ward Commissioner
- No Objection Certificate (Certified Copy) by Head of Household Ration Card of Parents for Inclusion of Additional Members

- Certificate of surrender in original (for inclusion, if any)
- Death certificate in case of death of a member
Documents required for change of head of household
- Original ration card
- Death certificate in case of death of the head of the family
- No objection to the certificate
Documentation required for conversion from child to adult
- Original ration card
- Proof of date of birth (birth certificate / X pass certificate / as declared / other)
Documentation required for FPS and/or change of residential address
- Original ration card
- Photo Identity / Electoral Identity Card / PAN / Driving License / Passport
Cardholder duties
- The ration card should be kept in safe custody and under no circumstances should this be given to any other person/PDS holder.
- While withdrawing ration, the cardholder shall check the quality/quantity and weight of the goods and he shall ensure that he receives the correct entry of the same made in the ration card as well as the cash memorandum.
- In case of loss/damage /alteration/addition/deletion of a ration card in the ration card entry, the cardholder shall immediately contact the concerned circle office. No additional writing /changes in their cards should be made by the cardholder.
- In case of the death of a family member and migration out of the state for a specified period, the name of the person should be deleted from the ration card.
